USAO Medical Form and Team Fees

I need to have everyone that is attending the USAO camp fill out the attached USAO Medical Release form and return it to me (or Sean) tonight. It also requires that you make a copy of your medical insurance card to attach to the form, so please do that as well.

I spoke with Oscar at Soccer USA and they will not require a deposit for the White uniforms, we will just have to pay for them when we pick them up. He is going to fax me a copy of the order form and I will have them tonight for you to fill out so that we can get the uniforms ordered.

With that said, I will need to collect a check made out to "OFC 98" (Mike C., correct me if I am wrong on that) for $157.50 for team fees. It breaks down like this...
- $60 for June 2010 fees (to establish account)
- $60 for August 2009 fees (due July 20)
- $37.50 for Beat the Heat Tournament (entry fee of $525/14 players)

I know this changed a bit, but the white team only ended up with 14 players as of today instead of the 15 I had originally figured cost with.

Monday Training Session

Parents,

There have been some slight changes to the tryout session on Monday. The session will only be a team and parent meeting, there will not be any on field activities, so the girls do not need to dress out in uniforms. Also, the session has been changed from 4:00pm to 4:30pm.

Tomorrow night we will collect the $155 for team fees. I will call Soccer USA during the day and check to see what we have to put down on the white uniforms and may have to add that to the total. Please check your daughter’s shirt and short size so you will know what you need to order.

As many of you know my daughter, Kelsie, was selected for the Black team therefore I will hand my manager duties over to Sean Berndt, Hannah’s Dad. Also, Mike Collison, Macy’s Dad, will be team treasurer. I am sure you will all be in very good hands. I would like to thank all of the team for being such a great group of parents and kids to have shared this season with, I have really enjoyed the time we have spent sweating, celebrating, anguishing, freezing, traveling, yelling, cursing (ok, maybe that was just me), coaching (from the sideline), laughing, and generally just hanging out together. It has been a lot of fun and has made for some great memories.

Team Fees News

I was notified today that our first training payment (August fees) will be due on July 20th. Therefore I have to collect an additional $60 at the Tryout Camp this weekend. The total you need to pay the team is now $155 + the cost of a new uniform, that I won't know the total of until Saturday.

$60 - June 2010 training fee (to establish a bank account)
$60 - Aug. 2009 training fee (due July 20)
$35 - Beat the Heat entrance fee
$?? - Uniform Fee

And, speaking of uniforms, the vote was for us to purchase the whole white uniform (shirts, shorts, and socks). If you haven't seen it, it does look really sharp.

Tryout Payment and Reminder Info

When making out your checks for tryouts on Saturday please follow these rules:

1) All OFC 98 East teams (that's us) will make their checks out to ESC.
2) One check for $235, dated July 29, 2009
or
3) Two checks - one for $135 dated July 29, 2009 and one for $100 dated Nov 1, 2009

All Players (including prior year Scholarship players and Board member's players) must turn in $235 for registration, except for the following exclusions:
-Contracted Coaches pay no registration fees for their children (Hampton, Phillips, Soesbee)
-All other Competitive Coaches pay $50 for Registration for each child playing (or can be 2 checks, $25each)

As a reminder tryouts are at the following times:
Saturday, 6/27 - 8:00 - 9:30am - OFC East Field 6
Saturday, 6/27 - 3:00 - 4:30pm - OFC East Field 6
Monday, 6/29 - 4:00 - 5:30pm - OFC East Field 7

It will be very hot at tryouts, please have the girls bring plenty of water for the day.

I will have packets with forms at the registration table at tryouts if you did not pick one up yesterday.

As a reminder you will need to have the following at registration:
- 2 copies of birth certificate
- 2 wallet size pictures (for player cards)
- Medical Release Form (signed)
- Code of Conduct Form (signed)
- USYSA Form (signed)
- $235.00 registration check(s) (write one check or split into two
- $135.00 payable July 31, 2009 and $100.00 post dated November 1st, 2009)
- $95 check (for June 2010 payment and BTH registration) made out to ESC 98 White.

Mike Collison has arranged for a notary that will notarize all of our medical forms, so there will be no need to have that done prior to registration. If you have any questions please let me know.

Tryouts - Uniform Vote

Tryouts are this weekend and I will have to collect some funds right away. As you know you will have to pay the registration fee of $235 at the first tryout session. You can pay the whole sum or you can break it up into two payments; $135 payable immediately and $100 payable November 1st.

We also need to decide if we want to order the white uniforms… several of the teams have decided to use this uniform. There are white shirts, white shorts, and socks. Or, we can just order shirts and wear with black shorts and socks. I will not know the cost until the day of tryouts but I imagine it will be close to what the others cost; $24 for shirt, $16 for shorts and $12 for socks. I guess we’ll put it to a vote – let me know by Thursday 5pm YES if you want to get the white uniform or NO if you don’t. Majority decides and if yes do you want the whole uniform or just the jersey. I would collect for these at tryouts

I will also need to collect the following fees at tryouts.
- $525 for the Beat the Heat Tournament = $35 per person (if 15 players on team)
- June 2010 coaching fees – this will be used to establish a team bank account.
- White uniforms – if we decide to order

total - $95 + uniform cost in addition to the registration fees.

Mike Collison has agreed to be the team treasurer and he may collect more between now and whenever Austin decides to start practice… I’ll gladly let him figure that out. ☺

I will be at ESC on Wednesday from 5:30 to 6:00pm if you would like to pick up a packet with all the forms you will need to fill out for tryouts… if you do not want to pick up a packet and would like to fill the info out at tryouts then please do not forget to bring the following.

- 2 copies of birth certificate
- 2 wallet size pictures (for player cards)

Academy Camp and Other Info

Below is an email from James Soesbee concerning Academy Camp. If you have
younger kids in Academy, or what to join Academy, then you'll want to read his
message.

Other news to note; Officially we took second in the Frontier Country league
this year. SLSC Cosmos Blue took 1st, which is the only team that beat us. All
in all, a pretty good season.

I will probably set up a time and place for everyone who wants to fill out their
tryout forms early to pick those packets up next week, but I'll let you know
when and where a little later.
